Practice Nurse

Background –

Menlo Park are working with a very supportive and well respected GP surgery, looking to add a Practice Nurse with chronic disease management experience to their dynamic, multi-disciplinary team. The practice is looking for a minimum of 2 days per week with the option to increase this if interested. This is a fantastic opportunity to join a well established team providing excellent patient care. With flexibility around start and finish times, working alongside other Practice Nurses and support with career progression and development, it really is a fantastic opportunity.

Salary – £33,000 – £40,000 per annum FTE + NHS pension + Agenda for Change annual leave + Bank Holidays

Location – Hull

The surgery –

• Very forward-thinking GP practice
• Regular clinical meetings to share best practice, etc
• Based at a single site
• Very well-respected within the local community
• Work with SystemOne computer system

Your role –

• Have primary care and chronic disease management experience
• Benefit from a very supportive MDT around you
• Be experienced with the traditional practice nurse duties
• Help out with enhanced access – am or pm
• Mainly face to face but some telephone calls

• Salary up to around £20 per hour DOE
